• Wrap Up/Remittance Form: Use this form to report final results to Community Health Charities.
  
• Tally Form for Large Organizations: Use this worksheet to subtotal results by department, floor, building, or other unit before completing the wrap up/remittance form. 
  
• Company Gift Form: Complete this form to provide documentation for a donation to Community Health Charities from your company.
  
• Sample Donor Thank You:  Use this modifiable template to send a thank you note or email to employee donors.
