Wrapping Up

Congratulations on completing your campaign! Your CHC staff representative will assist throughout the wrap-up process.

Download the following documents to complete the campaign process and ensure that CHC receives all necessary information.

• Wrap Up/Remittance Form: Use this form to report final results to CHC.
  
• Tally Form for Large Organizations: Use this worksheet to subtotal results by department, floor, building, or other unit before completing the wrap up/remittance form. 
  
• Company Gift Form: Complete this form to provide documentation for a donation to CHC from your company.
  
• Sample Donor Thank You:  Use this modifiable template to send a thank you note or email to employee donors.
  
• Campaign Recognition Awards: CHC presents awards to our campaign partners.  Let us know if you think you've earned some recognition for all your hard work!
